 ปัจจัยแห่งความสำเร็จ และทิศทางอนาคตงาน CBR ประเทศไทย

ความเป็นมา

การฟื้นฟูสมรรถภาพโดยชุมชน (Community-based Rehabilitation หรือ CBR ) เป็นแนวทางที่องค์การอนามัยโลก (WHO) ริเริ่มขึ้นหลังจากที่มีการประกาศใช้ปฏิญญาอัลมา-อตา (Declaration of Alma-Ata) ในปี ค.ศ. 1978 (พ.ศ.2521) เพื่อใช้เป็นยุทธศาสตร์ช่วยคนพิการในประเทศที่มีรายได้ในระดับต่ำและปานกลางสามารถเข้าถึงบริการฟื้นฟูสมรรถภาพต่างๆได้ดียิ่งขึ้น ด้วยการใช้ทรัพยากรในท้องถิ่นให้เกิดประโยชน์สูงสุด และด้วยความร่วมมือขององค์กรต่างๆ ของ สหประชาชาติ หน่วยงานภาครัฐ องค์กรพัฒนาภาคเอกชน และองค์กรเพื่อคนพิการคนพิการต่างๆ ตลอดระยะเวลากว่า 30 ปีที่ผ่านมา จึงทำให้ CBR มีการพัฒนาจนกลายเป็นยุทธศาสตร์ที่สามารถตอบสนองต่อความต้องการของคนพิการได้อย่างหลากหลายยิ่งขึ้น  ทำให้คนพิการมีส่วนร่วมและเป็นส่วนหนึ่งของสังคม  ตลอดจนยกระดับคุณภาพชีวิตของคนพิการให้สูงขึ้นอีก

CBR ได้ดำเนินการโดยทั้งภาครัฐและองค์กรพัฒนาเอกชน  โดยปี พ.ศ. 2522 – 2525 องค์การอนามัยโลก  ได้ริเริ่มฝึกอบรมและทดลองทำโครงการ CBR ในเขตภูมิภาคเอเชียและแปซิฟิก ซึ่งมีประเทศที่ได้เข้าร่วมโครงการ ได้แก่ อินเดีย อินโดนีเซีย พม่า ฟิลิปปินส์ และ ศรีลังกา ประกอบกับสหประชาชาติได้ประกาศให้เป็นปีคนพิการสากลในปี พ.ศ. 2524 ได้ส่งผลให้ประเทศต่างๆ ทั่วโลกเริ่มตระหนักถึงการฟื้นฟูสมรรถภาพคนพิการ สำหรับประเทศไทย กระทรวงสาธารณสุข ได้รับเอานโยบายการการฟื้นฟูสมรรถภาพภาพคนพิการในชุมชน  โดย พ.ศ. 2526 ได้ใช้ “คู่มือฝึกคนพิการ ของ องค์การอนามัยโลก” “Training Disabled People in The Community”  เป็นแนวทางในการฝึกคนพิการในชุมชน  ซึ่งกระทรวงสาธารณสุขได้มอบหมายให้คณะอนุกรรมการเวชศาสตร์ฟื้นฟู กรมการแพทย์เป็นผู้แปลและเรียบเรียง คณะอนุกรรมการชุดนี้ได้ทดลองใช้คู่มือนี้ที่อำเภอโนนสูง จังหวัดนครราชสีมา ในปี พ.ศ. 2529 และที่อำเภอวัดโบสถ์ จังหวัดพิษณุโลก ในปี พ.ศ.2530 หลังจากการทดลองใช้คู่มือและดำเนินการในหลายพื้นที่ จนได้ข้อสรุปว่า แนวทางการดำเนินการฟื้นฟูสมรรถภาพคนพิการในชุมชนของรัฐคือ “การใช้แนวงานของสาธารณสุขมูลฐาน” เป็นแนวทางในการดำเนินงานฟื้นฟูสมรรถภาพคนพิการในชุมชน   โดยมีศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ เป็นผู้รับผิดชอบหลัก

ทั้งนี้แนวคิด CBR ได้ขยายสู่วงกว้างในสังคมไทยทั้งภาครัฐและเอกชน เช่น กระทรวงศึกษาธิการ กระทรวงการพัฒนาสังคมและความมั่นคงของมนุษย์ รวมทั้งภาคเอกชน โดยในปี พ.ศ. 2528 มูลนิธิเพื่อเด็กพิการ ได้เริ่มนำร่องร่วมกับโรงพยาบาลบัวใหญ่ อำเภอบัวใหญ่ จังหวัดนครราชสีมา  จัดทำโครงการฟื้นฟูสมรรถภาพเด็กพิการในชุมชน ร่วมกันสำรวจ คัดกรอง เยี่ยมบ้านและดำเนินการฟื้นฟูสมรรถภาพคนพิการในชุมชน ปี 2531 ได้ขยายพื้นที่ ไปยัง โรงพยาบาลศรีบุญเรือง อำเภอศรีบุญเรือง จังหวัดหนองบัวลำภู  และปี พ.ศ. 2528 เช่นกัน มูลนิธิธรรมิกชนเพื่อคนตาบอดในประเทศไทยในพระบรมราชูถัมภ์ ได้ดำเนินการฟื้นฟูสมรรถภาพคน

ตาบอด โดยเฉพาะมุ่งเน้นด้านการศึกษาและการพัฒนาคนตาบอดในชุมชน รวมถึงหน่วยงานต่างๆ ที่ได้ร่วมกันบุกเบิกงานด้านการฟื้นฟูสมรรถภาพคนพิการโดยชุมชนในขณะนั้น เช่น  มูลนิธิธิดาเมตตาธรรม องค์การคริสโตเฟิลบลายเดินมิชชั่น (Christoffel-Blendenmission เรียกย่อว่า องค์การ CBM) องค์การแฮนดิแคป อินเตอร์เนชั่นแนล ประเทศไทย (Handicap International Thailand เรียกย่อว่า HI Thailand)

ปี พ.ศ. 2542 กรมประชาสงเคราะห์ กระทรวงมหาดไทย ได้ทำบันทึกข้อตกลงความร่วมมือ หรือ Memorandum of Understanding (MOU) กับองค์การ CBM ด้านการทำงาน CBR โดย องค์การ CBM ได้สนับสนุนกรมประชาสงเคราะห์ในการนำร่อง โครงการฟื้นฟูสมรรถภาพคนพิการโดยชุมชน 4 ภูมิภาค ทั้งด้านงบประมาณ และ องค์ความรู้ในการดำเนินการ

ปี พ.ศ. 2547 สำนักงานส่งเสริมและพัฒนาคุณภาพชีวิตคนพิการแห่งชาติ กระทรวงการพัฒนาสังคมและความมั่นคงของมนุษย์ ได้จัดทำ “โครงการพัฒนาศักยภาพคนพิการโดยชุมชน” โดยได้จัดอบรมอาสาสมัครพัฒนาสังคมและความมั่นคงของมนุษย์ช่วยเหลือคนพิการ (อ.พ.ม.ก.) เพื่อให้ อ.พ.ม.ก.ลงพื้นที่สำรวจคนพิการ จัดประชุมเครือข่ายเพื่อติดตามการดำเนินงานในชุมชน พัฒนาศูนย์เรียนรู้ จัดเวทีประชุมประชาคมเพื่อจัดทำแผนการส่งเสริมพัฒนาคุณภาพชีวิตคนพิการในชุมชน รวมถึง ผลักดันให้เป็นแผนปฏิบัติงานของท้องถิ่น ต่อไป

การดำเนินงานแนวคิด CBR ได้มีการดำเนินงานอย่างต่อเนื่องในประเทศไทย ทั้งนี้ เพื่อให้การดำเนินงานเกิดประสิทธิภาพและยั่งยืน ได้เคยมีการรวมกลุ่มระหว่างภาครัฐ ภาคเอกชนเป็นกลุ่มเครือข่ายที่มีศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติเป็นผู้ประสานงาน และ ริเริ่มจัดตั้ง

การปรับเปลี่ยนกระบวนการทำงาน CBR ในประเทศไทย

ระยะต่อมาโดยเฉพาะในช่วงประมาณ 20 ปีก่อนหน้านี้ เกิดการปรับตัว และเปลี่ยนแปลงในวงการ CBR ของประเทศไทยอย่างมากใน 2 ประเด็น ได้แก่ หนึ่ง แนวคิดการเสริมพลังคนพิการ (Empowerment) แนวคิดพิการต้องมีส่วนร่วมอย่างสมบูรณ์ (Nothing about Us without Us) จากระดับภูมิภาคและต่างประเทศ ส่งเสริมให้กลุ่มคนพิการ (Self-Help Group) รวมตัวกันและเป็นผู้จัดโครงการ CBR ด้วยตัวเอง ซึ่งส่งผลให้ CBR ในประเทศไทยพยายามหลุดจากแนวคิดเชิงการแพทย์ (Medical Model) มาเป็นแนวคิด เสริมพลังคนพิการ (Empowerment) และประเด็นที่สอง ที่ทำให้เกิดการเปลี่ยนแปลงและทำให้งาน CBR เป็นกระบวนการหนึ่งในงานพัฒนาชุมชน คือ การกระจายอำนาจบริหารงานส่วนท้องถิ่นสู่ระดับชุมชนของประเทศไทย คือ การเกิดองค์กรปกครองส่วนท้องถิ่นในระดับชุมชน ที่เรียกว่า องค์การบริหารส่วนตำบล (อ.บ.ต.) ที่ทำให้หน่วยงานภาครัฐและเอกชนเข้าไปทำงานร่วมกับ อ.บ.ต. และคนพิการในระดับหมู่บ้านและชุมชน และจัดทำโครงการ CBR ที่มี อ.บ.ต.เป็นเจ้าภาพหลักในการดำเนินงาน ซึ่งในปัจจุบัน พบว่า คนพิการและอ.บ.ต. ร่วมกันทำงาน CBR ได้อย่างเข้มแข็ง และมีประสิทธิภาพในระดับตำบลหลายพื้นที่

ปี 2556 กระทรวงพัฒนาสังคมและความมั่นคงของมนุษย์ กระทรวงสาธารณสุข และ องค์การ CBM ได้ร่วมมือกันแปลและจัดพิมพ์ CBR Guidelines ฉบับภาษาไทย ขององค์การอนามัยโลก

ล่าสุดในเดือนกรกฎาคม 2559 กลุ่มองค์กรเอกชนที่ทำงานด้าน CBR ผู้นำคนพิการ โดยการสนับสนุนจากผู้แทนภาครัฐในฐานะที่ปรึกษาในการดำเนินงาน ได้ริเริ่มรวมตัวกันเป็น  เครือข่าย CBR ประเทศไทย (Thailand CBR Network) ซึ่งเป็นเครือข่ายที่ระดมการมีส่วนร่วมของทุกภาคส่วนทั้งภาครัฐ ภาคเอกชน องค์กรด้านคนพิการ สถาบันด้านวิชาการ และองค์กรชุมชน ที่ปฏิบัติงานด้านการพัฒนาคุณภาพชีวิตคนพิการและครอบครัว โดยใช้หลักการแนวทางการทำงานแบบ การพัฒนาคนพิการโดยชุมชน (Community-based Rehabilitation: CBR ) เครือข่ายเน้นแลกเปลี่ยนเรียนรู้ พัฒนาองค์ความรู้ และกระบวนการทำงานเพื่อเชื่อมโยงระดับรากหญ้าถึงระดับนโยบาย เพื่อสร้างการเปลี่ยนแปลงที่ดีกว่าในกลุ่มเป้าหมาย ประสานสร้างความสัมพันธ์ระหว่างคนทำงาน และเคลื่อนไปสู่สภาเครือข่าย CBR ประเทศไทยในอนาคต ซึ่งจะเป็นเครือข่ายจัดตั้งเป็นทางการในระดับประเทศต่อไป

CBR ในระดับสากล

ในระดับสากล แนวคิด CBR ได้ถูกบรรจุในอนุสัญญาว่าด้วยสิทธิคนพิการ (Convention on the Rights of Persons with Disabilities) ในหัวข้อที่ 26 ที่ว่าด้วย การส่งเสริมสมรรถภาพ และ การฟื้นฟูสมรรถภาพ ข้อ (b) ที่เน้นการสนับสนุนให้คนพิการมีส่วนร่วมในชุมชน และ การเป็นส่วนหนึ่งในชุมชน และ ในทุกด้านของสังคม

ในเอกสารยุทธศาสตร์การทำงานของ UNESCAP ที่ใช้ชื่อว่า Incheon Strategy to “Make the Right Real” for Persons with Disabilities in Asia and the Pacific ได้เน้นย้ำในหลักการใหญ่ ทิศทางการทำงานเชิงนโยบายข้อ (i) เรื่อง การพัฒนาแบบครบวงจรที่มีครอบครัวและชุมชนเป็นศูนย์กลางจะช่วยพัฒนาคนพิการในด้านต่างๆ และโดยเฉพาะในโครงการขจัดความยากจน

ในส่วนของความร่วมมือในระดับภูมิภาคอาเซียน ในจุดเน้นที่ 5 ของเอกสาร ทศวรรษคนพิการอาเซียน ปี 2011-2020 (พ.ศ.2554-2563) เพื่อส่งเสริมการพัฒนาที่คนพิการมีส่วนร่วมในภูมิภาคอาเซียน ได้กล่าวถึง การดูแลสุขภาพ การบำบัดฟื้นฟู และ การฟื้นฟูคนพิการโดยชุมชน

ในช่วง 10 กว่าปีที่ผ่านมา ได้มีการก่อตั้งหน่วยงานระดับภูมิภาค ที่ กรุงเทพมหานคร ชื่อ ศูนย์พัฒนาและฝึกอบรมคนพิการแห่งเอเชีย-แปซิฟิก (Asia-Pacific Development Center on Disability: APCD) ซึ่งศูนย์ฯดังกล่าวได้มีส่วนร่วมพัฒนางาน CBR ผ่านการจัดฝึกอบรม และ ประชุมที่เกี่ยวข้องกับงาน CBR อย่างสม่ำเสมอในช่วงที่ผ่านมา

ปัจจุบันได้มีเครือข่ายหน่วยงาน องค์กรที่ทำงานด้าน CBR ในระดับต่างๆ ได้แก่ เครือข่าย CBR โลก เครือข่าย CBR ระดับทวีป ได้แก่ เครือข่าย CBR เอเชีย-แปซิฟิก อัฟริกา อเมริกาใต้ ฯลฯ ซึ่งทำให้มีการจัดประชุมแลกเปลี่ยนความรู้ ประสบการณ์ CBR ในระดับภูมิภาคในทวีปต่างๆ และในระดับโลก เช่น

  • การประชุม CBR ระดับโลก ครั้งที่ 1 เมือง อากรา ประเทศอินเดีย ในปี 2012 (พ.ศ.2555) และ การประชุม CBR ระดับโลกครั้งที่ 2 จัดที่ กรุงกัวลาลัมเปอร์ ประเทศมาเลเซีย ในปี 2016 (พ.ศ.2559)
  • การประชุม CBR เอเซีย-แปซิฟิก ครั้งที่ 1 ที่กรุงเทพฯ ประเทศไทย ในปี 2009 (พ.ศ.2552) ครั้งที่ 2 ที่กรุงมะนิลา ประเทศฟิลิปปินส์ ในปี 2011 (พ.ศ.2554) และครั้งที่ 3 ที่กรุงโตเกียว ประเทศญี่ปุ่น ปี 2015 (พ.ศ.2558)

ปัจจัยแห่งความสำเร็จ

จากการสรุปบทเรียน ผู้แทนองค์กรที่รวมตัวกันเป็น เครือข่าย CBR ประเทศไทยในช่วงเดือนกรกฎาคม 2559 ถึงประเด็นปัจจัยแห่งความสำเร็จในโครงการ CBR ของประเทศไทย ซึ่งสามารถรวบรวมสรุปได้ดังต่อไปนี้

  • ความร่วมมือ และการมีส่วนร่วมของทุกภาคส่วน ได้แก่
  • ความร่วมมือระหว่างภาคีเครือข่ายที่ดำเนินงานตามแนวคิดการทำงาน CBR Matrix

ทั้ง 5 องค์ประกอบ ตาม CBR Guidelines ของ องค์การอนามัยโลก (WHO) ได้แก่ ด้านสุขภาพ ด้านการศึกษา ด้านความเป็นอยู่ ด้านสังคม และด้านการเสริมพลัง

ข) การบูรณาการการทำงานร่วมกันในระดับนโยบาย และระดับปฏิบัติการ โดยมี

เป้าหมาย วิสัยทัศน์ ทิศทางเดียวกันในการพัฒนาคนพิการของแต่ละชุมชน และ ระดับชาติ

ค) การมีส่วนร่วมในการดำเนินงานของคนพิการ และของทุกภาคส่วน  โดยมุ่งเน้นให้

คนพิการ ครอบครัวสามารถพึ่งตนเองได้

  • มีการสร้างและพัฒนาเครือข่ายการทำงานร่วมกันกับระดับตำบล อำเภอ จังหวัด

และประเทศ ทั้งภาคประชาสังคม ภาคประชาชน ได้แก่ ครอบครัว อาสาสมัคร ชุมชน องค์กรคนพิการ (สภาคนพิการ สมาคมคนพิการ ประเภทต่างๆทั้ง 7 สมาคม) มูลนิธิเอกชน องค์กรธุรกิจเพื่อให้เกิดความเข้มแข็งและยั่งยืน สามารถขับเคลื่อนงานได้อย่างต่อเนื่องและเป็นจริงในชุมชน

  • การสร้างความเข้าใจ และ ความตระหนักของชุมชนและทุกภาคส่วน ได้แก่
  • สร้างความตระหนักรู้ว่า คนพิการเป็นสมาชิกหนึ่งในชุมชน คนพิการมีศักดิ์ศรีและคุณค่าแห่งความเป็นมนุษย์เหมือนกับทุกคนในชุมชน คนในชุมชนร่วมกันส่งเสริม และพัฒนาศักยภาพคนพิการ
  • ปรับทัศนคติของคนพิการและครอบครัวให้ ตระหนักรู้ถึงสิทธิ์และความต้องการของคนพิการเอง เน้นให้คนพิการมีอาชีพ มีศักดิ์ศรี พึ่งตนเองได้ ค) ผู้นำชุมชนท้องถิ่น เห็นความสำคัญของการทำงานเพื่อพัฒนาคนพิการ โดยการสร้างการมีส่วนร่วมของคนพิการ และการมีแผนการดำเนินงานเพื่อคนพิการอย่างยั่งยืน
  • การนำนโยบาย และกฎหมายมาใช้ให้เกิดผลในทางปฏิบัติ ได้แก่
  • การนำกฎหมายที่มีอยู่มาใช้ เพื่อสนับสนุนในทุกองค์ประกอบตาม CBR Matrix เช่น

การเสริมความเข้มแข็งของกลุ่มคนพิการ ระบบผู้ช่วยเหลือส่วนบุคคล ระบบการจ้างงาน การส่งเสริมอาชีพ ผ่านกลไกศูนย์บริการคนพิการ ตามมาตรา 20/3 และ 20/4 หรือ ผ่านมาตรา 33 (การจ้างงาน) และมาตรา 35 (การให้สัมปทานและการสนับสนุนด้านอื่นๆ) ตามพระราชบัญญัติส่งเสริมและพัฒนาคุณภาพชีวิตคนพิการ เป็นต้น

  • การผลักดันแนวงาน CBR เข้าสู่ศูนย์บริการคนพิการทั่วไป เพื่อให้เกิดการดำเนินการโครงการ CBR แบบองค์รวม และบรรจุงาน CBR เข้าไปในงานของศูนย์บริการคนพิการ เช่น สนับสนุนให้โรงพยาบาลส่งเสริมสุขภาพตำบล องค์การบริหารส่วนตำบล จัดตั้งศูนย์บริการคนพิการ โดยมีอาสาสมัครที่มีอยู่ในชุมชนดำเนินการ โดยอาจเริ่มจากการฝึกอบรม CBR ให้กับอาสาสมัครเหล่านี้ เพื่อให้สามารถดำเนินงานได้อย่างมีประสิทธิภาพ
  • การนำเอานโยบายของรัฐมาดำเนินการให้เป็นรูปธรรม เช่น นโยบายประชารัฐ ซึ่งเน้นการมีส่วนร่วมและความร่วมมือของภาคส่วนต่างๆ
  • การร่วมกันจัดทำแผนพัฒนาคุณภาพชีวิตคนพิการในระดับประเทศ จังหวัด และ

ชุมชนที่สามารถนำไปปฏิบัติได้จริง เช่น การทำงานร่วมกับ คณะอนุกรรมการจังหวัด การทำงานร่วมกับ องค์การปกครองส่วนท้องถิ่น (องค์การบริหารส่วนตำบล องค์การบริหารส่วนจังหวัด ฯลฯ) โดย มีการทำบันทึกความเข้าใจ หรือ ข้อตกลงร่วม ที่เป็นทางการร่วมกัน

  • การริเริ่มงาน และการทำงานเชิงรุก ได้แก่
  • การทำงานควรเริ่มจากระดับตำบล เพราะเป็นหน่วยที่ดูแล บริหารจัดการได้ง่าย ในการเริ่มต้น และโครงสร้างทางกฎหมายเอื้อต่อการทำงาน
  • ควรเริ่มจากภาคสาธารณสุข เนื่องจากคนพิการส่วนใหญ่จะต้องเข้าสู่กระบวนการฟื้นฟูสมรรถภาพและการดูแลสุขภาพก่อนจะเข้าสู่กระบวนการพัฒนาในด้านอื่นๆ จึงเป็นจุดเริ่มต้นที่สะดวกในการพัฒนาต่อ
  • การถ่ายทอดความรู้

ควรจัดให้มีเวทีแลกเปลี่ยนเรียนรู้ร่วมกัน รวมถึงการจัดให้มีการสรุปบทเรียน การถ่ายทอดองค์ความรู้ และการพัฒนาระบบการทำงาน อย่างมีประสิทธิผลและอย่างเป็นรูปธรรมต่อเนื่อง สม่ำเสมอ

  • การระดมทรัพยากร และการสนับสนุนจากภายนอกชุมชน ได้แก่
  • การประสานทรัพยากรในชุมชนที่มีประสิทธิภาพ ต้องเชื่อมให้ชุมชนได้มีส่วนร่วมโดยตรง และมีการดำเนินการใช้ทรัพยากรให้เกิดประโยชน์และประสิทธิภาพมากที่สุดต่อการดำเนินงาน
  • การสนับสนุนที่เข้มแข็งด้านทรัพยากร จากหน่วยงานและองค์กรต่างๆภายนอกชุมชน
  • การสร้างความสัมพันธ์ระหว่างภาคี ได้แก่การมีสัมพันธภาพที่ดีกับภาครัฐ และภาคีต่างๆ เพื่อหนุนเสริมการทำงานร่วมกันอย่างมีประสิทธิภาพ และช่วยทำให้เกิดความยั่งยืนในระยะยาว
  • การเผยแพร่ประชาสัมพันธ์รูปธรรมความสำเร็จ ได้แก่ การนำรูปธรรมแห่งความสำเร็จ

มาเผยแพร่ประชาสัมพันธ์ให้สังคมวงกว้างรับรู้ร่วมกัน เช่น การที่คนพิการมีอาชีพ มีรายได้ ถือเป็นการคืนศักดิ์ศรีให้แก่คนพิการ พ่อแม่เด็กพิการสามารฟื้นฟูลูกพิการให้ช่วยตนเองและอยู่ในสังคมได้

ทิศทางของงาน CBR ในอนาคต

ผู้แทนขององค์กรเครือข่าย CBR ประเทศไทยได้มีข้อเสนอถึงทิศทางในอนาคตของ CBR ประเทศไทยไว้ ดังนี้

 Successful Factors and Future Directions of Thailand CBR

 

Background

Community-based Rehabilitation (CBR) was initiated by the World Health Organization (WHO) following the Declaration of Alma-Ata in 1978. It was promoted as a strategy to improve access to rehabilitation services for people with disabilities in low-income and middle-income countries, by making optimum use of local resources. Over the past 30 years through collaboration with other UN organizations, governmental, non-governmental organizations and disabled people’s organizations, CBR has evolved into a multi-sectoral strategy to address the broader needs of people with disabilities, ensuring their participation and inclusion in society and enhancing their quality of life.

In Thailand CBR has been implemented by governmental and non-governmental organizations. Historical events related to CBR implementation in the region and Thailand are highlighted as follows:

  • 1979-1982 WHO initiated a training and pilot project of CBR in the Asia

and Pacific region, the countries involved were India, Indonesia, Burma,  the Philippines, and Sri Lanka.

  • 1981 the United Nations (UN) declared the year 1981 as the International

Year of Persons with Disabilities.

  • 1983 the Thai Ministry of Health applied the policy of CBR and utilized the

WHO manual “Training Disabled People in the Community” with persons with disabilities in the community.

  • 1985 NGOs such as the Foundation for Children with Disabilities and the

Christian Foundation for the Blind in Thailand started implementation of CBR in Thailand. Later many non-governmental organizations including the Daughter of Charity, CBM, Handicap International, etc. implemented CBR projects in their specific target areas.

  • 1999 the Department of Social Welfare, Ministry of Interior and CBMsigned a Memorandum of Understanding (MOU) for collaboration on implementation of CBR. CBM supported with knowledge and finance for CBR implementation at 4 pilot regional areas in Thailand.
  • 2004 the Thai National Office for Empowerment of Persons withDisabilities and the Ministry of Social Development and Human Security implemented the ‘Project on Development of Persons with Disabilities by Community’. The project trained and supported volunteers to survey persons with disabilities, conduct meetings with networks, develop learning centers, and organize public hearings for formulation of plans to develop the quality of life of persons with disabilities.
  • A network among governmental and non-governmental organizations wasinitiated and coordinated by the Sirindhorn National Medical Rehabilitation Center.

Turning Points of CBR Implementation in Thailand

Within the last 20 years there have been changes and adaptations in Thailand CBR activities. Two major points are firstly, the concept of “Empowerment” and “Nothing about Us without Us”. The concepts were transferred from the international level and assisted CBR in Thailand to go beyond the medical model and embrace the empowerment model. Secondly, the delegation of a new local administration system was done and included the establishment of Sub-district Administrative Organizations (SAO) as local government in community. Many governmental and non-governmental organizations began to work with SAOs on social development in the community including CBR. Presently there are many SAOs working with persons with disabilities at the sub-district level and they are using the CBR concept effectively.

In 2013 WHO CBR Guidelines were translated and printed in Thai language and shared throughout Thailand as a contribution by the Ministry of Social Development and Human Security, the Ministry of Health and CBM.

In July 2016 the Thailand CBR Network was established by representatives of non-governmental organizations and disabled people organizations with advisory support from representatives of governmental organizations. Thailand CBR Network purpose is to mobilize participation from all sectors including governmental, non-governmental, disabled people organizations, academic institutes and other community organizations. The Network focuses on exchanging ideas and learning, developing knowledge and working processes for links between grass roots and policy levels. The Network aims to improve the lives of target groups, coordinate CBR workers, and develop an official CBR network federation in Thailand in the future.

CBR at the International Level

CBR concept and principles are a  key strategy  in  international instruments  including the Convention on the Rights of Persons with Disabilities (CRPD), Incheon Strategy to “Make the Right Real” for Persons with Disabilities in Asia and Pacific, and the  ASEAN Decade of Persons with Disabilities (2011-2020).

One regional office the Asia-Pacific Development Center on Disability (APCD) was established in Bangkok, Thailand more than 10 years ago. APCD has contributed to the development of CBR by conducting trainings and meetings regarding CBR regularly.

There are many networks of organizations working on CBR at different levels, for example, CBR Global Network, CBR Asia-Pacific Network, and CBR Africa Network. The Networks organize congresses for exchanging knowledge and experiences about CBR at the continental and global levels;

  1. CBR World Congress: the first congress was held in Agra, India (2012) and

the second congress was in Kuala Lumpur, Malaysia (2016).

  1. Asia-Pacific CBR Congress: the first congress was held in Bangkok, Thailand

(2009), the second congress was in Manila, Philippines (2011) and the third congress was in Tokyo, Japan (2015).

Successful Factors of CBR 

Based on reflection of lessons learned of representatives of organizations who formed the Thailand CBR Network in July 2016, the successful factors for CBR were identified;

  1. Collaboration and Participation of All Sectors
  2. Collaboration among networks and sectors which implement CBR according

to the 5 components of the CBR matrix as mentioned in the WHO CBR Guidelines (Health, Education, Livelihood, Social and Empowerment).

  1. Integration of CBR policy at the  implementation level among organizations

which have common goals and visions on the development of persons with disabilities at the community and  national level.

  1. Participation in CBR implementation by persons with disabilities in all sectors

with the main purpose being persons with disabilities and their families helping themselves in the community.

  1. Developing networking and collaboration at the community, district, provincial,

and national levels with participation of people with disabilities and their families, volunteers, community organizations, disabled people organizations, non-governmental organizations, and the private sector in order to strengthen and sustain CBR activities.

 

  1. Raising Awareness of the Community and All Sectors
  2. a) Raising awareness that persons with disabilities are part of the community and

that they have their own dignity as others. Encouraging the community to promote the potential of persons with disabilities.

  1. b) Adjusting attitudes of persons with disabilities and their families in order to be

aware of their rights and needs, with an emphasis on having incomes, dignity and being independent.

  1. c) Community leaders are aware of working with persons with disabilities and

they promote participation of persons with disabilities in the formulation of sustainable development plans.

  1. Application of Policy and Regulations for Practical Implementation
  2. a) Application of existing regulations to support CBR implementation according

to the CBR Matrix such as strengthening self-help groups of persons with disabilities, personal assistant system, employment system, jobs promotion through Disability Service Centers according to article 20/3 and 20/4, as well as Articles 33 (Quota system for employment) and 35 (grant concessions with other supports) according to the Thai Action Empowerment of Persons with Disabilities.

  1. b) Integrating the CBR approach and activities to Disability Service Centers.

Encouraging District Hospitals and Sub-district Administrative Organizations to set up Disability Service Centers where the volunteers are trained and supported to implement CBR in their communities.

  1. c) Applying the current Thai central government’s policy which is promoting collaboration between citizens and the state in all aspects.
  2. d) Collaboration on the formulation of plans for the development of the quality of

life of persons with disabilities at the national, provincial and community levels, which could practically be applied to the Provincial Sub-committee for Empowerment  of Persons with Disabilities and the local administrative organizations at the  provincial and sub-district levels by developing Memorandums of Understanding (MOUs).

  1. Being Pro-active and Showing Initiative
  2. a) CBR could start from the sub-district level as it has the most potential, and is

not too difficult to manage. There are also existing available regulations.

  1. b) CBR could start from the health component of the matrix because most

persons with disabilities are treated in the rehabilitation and health care process before becoming involved with other components. The health component has the most potential to start up CBR.

  1. Transferring of Knowledge

Conducting forums for knowledge sharing, lessons learned reflection, knowledge transfer, and how to improve working process regularly, and effectively with concrete outcomes.

  1. Resource Mobilization and Support from External Organizations
  2. a) Mobilizing of resources and developing direct links to community for the most

effective resource utilization.

  1. b) Strong support with resources from external organizations.
  1. Healthy Relationships Among Different Sectors

Healthy relationships with governmental organizations and other agencies will

support working together effectively and sustain projects into the future.

  1. Promote Successful Cases/ Stories by Sharing with the Public

By disseminating successful cases/ stories of persons with disabilities their families and communities, for example, persons with disabilities who have decent jobs and income, parents who can manage rehabilitation for their children with disabilities and can support themselves in society as others.

Future Directions of Thailand CBR

The representatives for organizations of the Thailand CBR Network proposed the following future directions;

  1. Promotion, setting up and development of a stronger and more sustainable

CBR network at different levels (community, provincial, national and regional levels). The networks could link between grass roots and policy levels and facilitate forums to develop and share lessons learned by applying, extending and linking with with other sectors.

  1. Organizing annual national CBR forums in order to motivate sharing and a

learning atmosphere with a goal to improve working processes.

  1. Link with organizations who work on health policy such as the National Health Commission Office of Thailand in order to expand the CBR approach.
  2. Application of existing regulations to support CBR implementation such as strengthening self-help groups of persons with disabilities, personal assistant system, employment system, jobs promotion through Disability Service Centers and Article 33, 35 of the Thai Act on Empowerment of Persons with Disabilities. Integrate the CBR approach and activities with Provincial and general Disabilities Service Centers, by applying the central government’s policy on encouraging participation and collaboration between citizens and state.
  3. Collaboration on formulation of plans for the development of quality of life for persons with disabilities at the national, provincial and community levels by working with the Ministry of Health and Ministry of Social Development and Human Security to integrate the CBR approach. Collaboration with provincial sub-committees for the improvement of quality of life of persons with disabilities by developing provincial plans and finally by working with local administrative organizations at the provincial and sub-district levels by developing Memorandums of Understanding (MOUs).
  4.   Sharing and communicating concrete success models/ stories to extend and document that  CBR in Thailand develops the community.

Remarks* The Thailand CBR Network was established on the 5th July 2016 as a network to mobilize participation of all sectors; governmental, non-governmental and disabled people organizations, academic institutes, and community organizations which are working with the CBR approach to develop quality of life of persons with disabilities and their families.

The Network focuses on sharing lessons learned, developing knowledge and working process for links between grass roots and policy levels for positive change for target groups by coordinating healthy relationships among CBR workers.

The Network will move to an official CBR federation at the national level.

Present committee members are composed of representatives of non-governmental organizations, disabled peoples organizations, and are supported by advisors, Director of Sirindhorn National Medical Rehabilitation Institute, Ms.Vijita Rachatanantikul, Director of Strategies and Plans Division, Department of Empowerment of  Persons with Disabilities, and Ms. Mayuree Pewsuwan.
